A weekly client check-in template for personal trainers

A useful weekly check-in helps you decide what to discuss or adapt. Keep it short enough for the client to complete and ask only questions you intend to review.

Five questions to copy and adapt

  1. How many of your planned sessions did you complete?

    Use a numeric answer from zero to the number of sessions you agreed. State the reporting dates so the client knows which week you mean.

  2. How manageable did the programme feel this week?

    Use a scale from 1 to 5. Label 1 as “not manageable” and 5 as “very manageable”. Keep the wording and scale consistent between check-ins.

  3. Which exercise or instruction would you like help with?

    Use optional free text. Let the client identify a specific exercise rather than requiring a general rating for everything.

  4. What made it easier or harder to follow the plan?

    Use optional free text. This can reveal practical barriers such as time, equipment or unclear instructions.

  5. What would you like us to focus on next?

    Use a short text answer to give the client a voice in the next conversation.

Set expectations before sending

Explain when you normally read responses and how clients should contact you when they need a quicker reply. A weekly form should not imply immediate monitoring. Avoid collecting sensitive information that you do not need for the agreed service.

Turn answers into a follow-up

  1. Read the answers together rather than judging a single score in isolation.
  2. Identify one question to discuss or one instruction to clarify.
  3. Record your follow-up in private client notes and update the programme where appropriate.
  4. Keep the core questions stable long enough to compare responses; remove questions that never change your decisions.
